CSCH Introduces New Nurse Navigator Program
Centra Southside Community Hospital introduces Susan Copsetta, RN, MSN, CDE (Certified Diabetes Educator), CPT (Clinical Insulin Pump Trainer), ACNS-BC, as the leader of the New Nurse Navigator program. A Nurse Navigator is a member of the healthcare team whose role is to assure that the patient receives the right care at the right time. A nurse navigator proactively assists the patient in managing their healthcare resources by helping them "navigate" and overcome barriers to access quality care. These resources may be related to medical care, financial assistance, counseling, language needs, or transportation.
Typical tasks of the Nurse Navigator include, finding a specialist; scheduling appointments; referring patients to community resources; providing patient education and screening information and helping patients to understand medical terminology.
